• A valid e-mail address will be the primary method the Department will use to communicate with you.
• A primary telephone number: This should be a personal telephone number that the Department can use to contact you about your medical marijuana registration.
• Proof of Identity or Proof of Connecticut residency 

4. Our Connecticut State Certified Doctors to establish whether the patient qualifies for Medical Marijuana under the CT Compassionate Use of Medical Marijuana statutes.
